You have booked a course and been sent a link asking for some details and a signature. Here is what all of that is for.

What is the link I was sent?

It is your own personal form — one per person on the booking, so if you booked for two people there are two links and each person fills in their own. It saves as you go, so you can close it and come back.

Why am I being asked medical questions?

Freediving is breath-hold diving, and a few medical conditions carry more risk underwater than they do on land. The questions come from your training agency, not from your dive school, and every school that teaches the course asks the same ones.

Answering yes to something does not usually mean you cannot dive. It means a doctor should say so first.

I answered yes to something. What happens now?

Your school will get in touch. In most cases you will need a doctor to sign a short form confirming they see nothing that makes freediving unsafe for you — and then you dive as normal.

Do this early rather than the night before. Getting a doctor's appointment is the slow part, not the form.

If you are unsure whether something counts, answer yes. The form is designed for that — an unnecessary conversation with a doctor costs you an afternoon, and the other way round costs more.

Who sees my answers?

Your medical answers go to your dive school, and within it only to the people who need them. Instructors and crew working the day see whether you are cleared to dive — not what you answered.

What am I signing?

Usually two things: a liability release, and a declaration that your medical answers are honest and complete.

You will see the full document, with your details already filled in, before you sign anything. Read it. If something in it is wrong, stop and tell your school rather than signing and mentioning it later.

I am under 18

A parent or guardian has to sign for you. They should be the one filling in the signing page — their name goes on it, not yours.

When am I charged?

That depends on your school. If you paid a deposit, the rest is usually charged automatically a set number of days before the course starts, using the same card — you agreed to this when you booked, and the exact number of days was shown at checkout.

If a payment does not go through, you will be emailed with a link to pay rather than simply losing the booking.

My link does not work

Links expire, and each one only works for a single booking. Reply to the email from your dive school and they will send a fresh one. Do not use somebody else's link — you would be filling in their form.

Cancelling or moving my booking

Cancellation terms are set by your dive school and were shown when you booked. Contact them directly — they can move you to another date or process a refund under their policy.
